Each month, you choose the services that fit your needs. No two months ever have to be the same. Whether you are recovering from an injury, managing stress, improving movement, or staying proactive about your health, your membership gives you the freedom, savings, and consistency to feel your best all year long. If additional treatments are required and/or desired, then members receive 10% off the additional visits, $94.50. Our normal fee for these services is $105.00.

Yes. The membership requires a 12-month commitment to help you stay accountable to your health goals. If you decide to cancel early, the total value of the remaining months will be billed as a one time fee at a 10% discount.
ALLIED HEALTHCARE MEMBERSHIP TERMS & AGREEMENT
Your Allied Healthcare Monthly Membership includes your choice of services each month. The number of visits you receive depends on the membership option you select.
MEMBERSHIP OPTIONS
Essential Care Membership: $99 per month – includes 1 visit per month
Complete Care Membership: $189 per month – includes 2 visits per month
Family Care Membership: $369 per month – includes 4 shared visits per household
All memberships include access to the following services:

60-Minute Massage Therapy
Acupuncture
Neurofeedback
Nutritional and Lifestyle Counseling

MEMBERSHIP CONDITIONS
The number of visits included each month is determined by the membership selected.
Included visits must be used within the same month. Visits cannot roll over, be saved, or banked for future use.
Membership fees are automatically drafted each month and applied toward your selected services.
The Family Care Membership allows all household members to share the included visits.
The Essential and Complete Memberships are for individual use only.
Membership requires a 12-month commitment to support consistent care and progress.
Early cancellations before completing the 12-month term will result in billing for the remaining balance at a 10% discount on the total remaining months.
Members must be at least 18 years old. Members under 18 must have parental consent.
Provider gratuity is not included in membership pricing and is appreciated at the time of service.
Allied Healthcare reserves the right to adjust service offerings, pricing, or terms with a minimum of 30 days’ written notice.
CANCELLATION POLICY
Membership may be canceled after completing the 12-month term with 30 days’ written notice sent to office@myalliedhealthcare.com.
Early cancellations before 12 months will result in a charge for the remaining term, as outlined above.
If there are any issues processing payment, it is the member’s responsibility to resolve the balance within 3 business days.
Memberships automatically renew annually at the same rate unless canceled in writing 30 days before renewal.
RECURRING CHARGE / AUTOMATIC PAYMENT ACKNOWLEDGMENT
The recurring charge covers the cost of your selected membership plan ($99, $189, or $369).You authorize Allied Healthcare to charge the card on file on the 1st of each month for the duration of your 12-month commitment.
Payments are processed through a secure, encrypted payment system.
In compliance with Colorado law, a 3% merchant processing fee applies to all credit card transactions (debit cards excluded).
It is the member’s responsibility to notify Allied Healthcare of any changes to their payment method.
Members will receive a receipt for each transaction. Notification will be provided in advance of any change in amount or draft date.
TERMINATION OR SUSPENSION OF MEMBERSHIP
Allied Healthcare reserves the right to cancel or suspend a membership if:
The member breaches any terms of this agreement, or
Payments remain unpaid 30 days past due.
ACKNOWLEDGMENT & ACCEPTANCE
By signing this agreement, you confirm you have read, understood, and agreed to all terms of the Allied Healthcare Monthly Membership.
